Updating EOSY Status of Honor Learners in LIS Website Please be advised of the additional options in updating EOSY status of learners: Promoted with Honor Promoted with High Honor Promoted with Highest Honor Applicable for G1 to G10 only. For updating of EOSY status of honor learners, here are the steps: 1. Go to LIS website http://lis.deped.gov.ph/ and log in using the LIS User account. Once login, click the Learner Information System Link. Upon accessing the LIS, click List of Classes tab. On the desired Grade Level and Section, click Review and Finalize. Search the student’s name in the list and click the Pencil icon beside No Status. Click Status dropdown list. The following status were added: Enter General Average (whole numbers only). If applies, tick ALIVE and CTC checkboxes. Click Update . On successful submission, the learner’s status should be updated showing the General Average. Ano po ba ang sinasabi ng DepEd Order No. 11, s. 2018 - Guidelines on the Preparation and Checking of School Forms? 1. Only School and Division levels checking is allowed. 2. There will be NO District Level checking of forms. 3. Only SF1, SF4, SF5 and SF10 will be subjected to checking. 4. NO additional forms shall be required for checking. 5. Only LIS Generated forms are acceptable. Commercialized forms is not allowed. 6. Source of Funds: School MOOE or school canteen funds for SCC and SDO MOOE for DCC. School Forms Checking Report (SFCRs) per Adviser

SFCR per Adviser will be used or filled up by the SCC for consolidation and to determine level of accuracy of the adviser and school and by the DCC for the final and official level of accuracy of the school. It is the SCC who will use or fill up the SFCR per adviser and not the adviser. Advisers are not going to prepare SFCR. The school will print SFCR for adviser to be used and filled up by the SCC to be consolidated in SFCR1 annexed to DO11,s.2018. 4th Quarter Daily Lesson Log (DLL) - Grade 1 To enable the teachers to do other meaningful teaching-related tasks, which include, but not limited to preparing instructional aids, assessing learners’ portfolios and conducting learning interventions, the following Guidelines on the Preparation of Daily Lessons (DLs) are issued: a. Teachers who have been in the service for more than two (2) years, private school experience included, shall not be required to prepare detailed lesson plans (DLPs). They may adopt the Daily Lesson Logs (DLLs) which contain the following entries: Lesson as cited in the TG/TM reference materials with the page/s number; Learners’ Material used such as Activity Sheets, Modules, other materials with the page number reference; Remarks indicating number of learners within mastery level; number of learners needing enrichment/refinement lessons; and Other activities include the interventions given to the pupils/ students who did not master the lesson(s) in the previous
